To start vibrations it is best to strike the middle of the tuning fork a sharp blow against the side of your hand. Alternatively the ends of the tuning fork can be “pinged” between the thumb and forefinger.

Rydel Seiffer Tuning Fork

When vibrating, two triangles appear on the graduated scale of 0-8 that join together as the amplitude decreases. When the patient says they are unable to feel the vibrations, the scale is read at the apex of the single triangle formed from the initial two triangles.

Two scales are provided, black and white. They work in the same way and it is largely a matter of user preference as to which one is used. However, it is recommended that the same scale is used for repeat assessments.


To Use The Scales

First set the tuning fork as above and then watch for the black or white triangle to start forming at the bottom of the scale. At this point the tuning fork should be applied to the patients foot. If the patient can feel the vibration he/she is asked to say when they can no longer feel it. As the vibration dies away the triangle enlarges and the tip makes its way to the top of the scale. The scale can then be read off at the point at which the patient can no longer detect the vibration.

The graduation level 4 approximates to a level of 20-25 volts on a biothesiometer which is the generally used screening level for protective sensation. If a patient cannot feel the vibration at all they have lost protective sensation and are definitely at risk.

Because vibratory sensation tends to weaken naturally with age this test should not be relied upon as the only determinant of protective sensation in elderly patients.

Note: The precise positioning of the weights is critical to the instruments function and they should not be adjusted for any reason.
